No Device Fees or Upfront Costs — Insurance Billable
Affordable Access to Ongoing Healthcare Support
Community Strong LLC does not charge patients a separate device fee or require an upfront payment for monitoring equipment provided as part of their eligible program.
Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Behavioral Health Integration (BHI) are healthcare services that may be billed to the patient’s insurance when medically necessary and when coverage requirements are met.
We work with:
- Medicare
- Medicaid
- Medicare/Medicaid Dual-Eligible Plans
- Most Commercial Insurance Plans
Patient costs depend on individual insurance coverage. Deductibles, copayments, or coinsurance may apply. Patients with Medicaid, secondary/supplemental insurance, or certain Medicare Savings Program coverage may have little or no out-of-pocket responsibility.
Insurance benefits and eligibility are verified before enrollment, and patients are informed of any known cost-sharing responsibility.
Our services help patients stay connected with their healthcare team, communicate how they are feeling, share their needs, and receive ongoing support between healthcare visits.
